Complete your Health Risk Assessment for 2021 by December 30th. If you haven't received your bloodwork yet, you'll want to contact your health care provider to see if you can get it done before the year is out. Once you have your cholesterol, triglyceride, glucose, and blood pressure results, you may click here to fill out the Assessment. Qualifying participants who complete the Assessment may receive $70 cash back. Qualifying participants are employees or spouses who are on a DMBA health insurance plan through BYU.
