Screening & HRA - $70/Year

Eligible participants for the Living Healthy Wellness program include non-student employees and their spouses who have a DMBA health insurance plan through BYU. Each eligible participant who gets a Triple Screening and takes DMBA's Health Risk Assessment (HRA) may receive a $70 premium reimbursement on the employee's paycheck at the beginning of the following year.

(For example, if you are the employee and you and your spouse both fill out the assessment in January 2022, $140 would appear on the your paycheck by the end of March 2023.)

Triple Screening

In order to fill out the HRA, you will need to have your cholesterol, glucose, and triglyceride numbers, as well as your blood pressure. This generally involves taking your blood pressure and either a finger prick or blood draw.

To obtain this bio-medical information*, you may do one of the following:

1. Sign up for a Triple Screening from the Student Health Center.

Appointments are required to get a triple screening from the Student Health Center. You can get your triple screening done at any point during the year, but we encourage you to go during your birth month or before November each calendar year.

Appointments are generally available Monday, Tuesday, and Thursday 8 am - 11 am and Fridays 9 am - 11 am.

If you have never visited the Student Health Center before, please call 801-422-5156 to register as a patient and schedule your appointment.

If you are already a registered patient at the Student Health Center (you are a patient if you have received a triple screening there before), you are welcome to click here to schedule an appointment online. You will need to select the "Wellness labs" service. If the link above is not working, call the Student Health Center to make an appointment at 801-422-5156.

2. Make an appointment for a triple screening with your own medical provider.

If you cannot go to the Student Health Center, most medical providers have the ability to do this triple screening for you.

*If you have had your blood work done (within the year), you don't need to do it again.

Health Risk Assessment

Once you receive your Triple Screening results, you will want to fill out the Health Risk Assessment (HRA). Click here to take the HRA on DMBA's website.

One you submit the assessment, you'll receive a report regarding your health risks. By taking steps to mitigate these risks, you can improve your health and well-being.
